Receiving advanced care at home involves setting up a command center to serve as a virtual hospital ward and checking in with medical teams through a tablet. This type of advanced care at home is ideal for people with heart failure, pneumonia, cellulitis or chronic obstructive pulmonary disease exacerbations.
